What is MyVaxIndiana?
The MyVaxIndiana Immunization Portal is an app created by the Indiana State Department of Health (ISDH) to enable individuals to access their vaccine records from any computer. The app is an update to the existing process and is run under the Indiana Code 16-38-5, which allows registered medical providers in Indiana to collect, forecast, manage, and share vaccine data through the Children and Hoosier’s Immunization Registry Program (CHIRP). The app allows individuals to request a patient ID number (PIN) from a registered CHIRP provider to access their personal or their dependents' vaccine records.
