What is Uw Zorg online?
Uw Zorg app is a service provided by your general practitioner that allows you to access your medication overview, order previously prescribed medications, make appointments, and start an eConsult with your GP 24/7. The app also provides a list of affiliated practices. Users can provide feedback to improve the app via the feedback button or by emailing apps@pharmeon.nl. The app requires users to create an account and a 5-digit pin code to ensure privacy.
1. Hiermee heeft u 24 uur per dag toegang tot uw medicatieoverzicht, kunt u eerder voorgeschreven medicijnen bestellen, afspraken maken en een eConsult starten bij uw huisarts! Een overzicht van aangesloten praktijken vindt u in de app.
2. Als u al een account heeft voor een patiëntportaal bij uw huisarts logt u met dezelfde gegevens in op de app en gaat u direct naar stap 4) .
3. Als u nog geen account heeft, vraagt u die bij ons aan door op de knop ‘Aanmelden’ te klikken en alle benodigde gegevens in te vullen.
4. Met de app kunt u via een beveiligde verbinding uw medicatie gegevens ophalen uit het systeem van de praktijk en communiceren met uw arts.
5. • Stel uw medische vragen direct aan uw arts via een eConsult en ontvang bericht zodra uw consult beantwoord is.
6. Voor de ingebruikname wordt uw identiteit eerst geverifieerd door de praktijk en ontvangt u een verificatiecode om de app te activeren.
7. Let op! eConsult is niet bedoeld voor urgente zaken of levensbedreigende situaties.
8. • Bekijk de lege plekken in de agenda van uw arts en boek een afspraak op het moment dat het u uitkomt.

10. • U vindt de adresgegevens, contactgegevens en openingstijden van uw arts in de app.

- Access to your current medication profile as known by your GP
- Request repeat prescriptions directly from your medication list and receive reminders when you need to refill your medication
- Ask medical questions directly to your GP via eConsult and receive a response when your consultation is answered. Note that eConsult is not intended for urgent matters or life-threatening situations. If you are unsure about the severity of your condition, always contact your GP by phone.
- View available slots in your GP's schedule and book an appointment at a time that suits you. You must also specify the reason for your appointment.
- Find the address, contact information, and opening hours of your GP in the app. You will also find a link to your GP's website.
- The app uses a secure connection to retrieve your medication data from the practice's system and communicate with your GP. Before use, your identity is verified by the practice, and you receive a verification code to activate the app. Additionally, users must secure the app with a personal 5-digit pin code.
