What is Docket?

Docket® is a mobile app that partners with public health departments to provide easy access to personal and family immunization records. Users can track upcoming shots, review past immunizations, and share official immunization reports. The app also offers a SMART® Health Card vaccine credential QR code for residents of Alaska, Idaho, New Jersey, and Utah to facilitate travel to places with COVID-19 restrictions.



Software Features and Description

1. Docket® partners with public health heroes to make it easier than ever for you to access personal and family immunization records directly from Alaska Department of Health, Idaho Department of Health and Welfare, Minnesota Department of Health, New Jersey Department of Health, and Utah Department of Health and Human Services.

2. Alaska, Idaho, New Jersey and Utah residents can access a SMART® Health Card vaccine credential QR code to facilitate travel to places with COVID-19 restrictions.

3. NOTE: You must have a valid email address and/or phone number on file with your state's immunization registry in order to access personal or family immunization records using this app.

4. Please consult with your health department if you have trouble locating your records using this service.

5. Track upcoming shots, review past immunizations, and share your official immunization reports.

Install Docket® - Immunization Records app on your Windows in 4 steps below:


  1. Download an Android emulator for PC:
    Get either Bluestacks or the Nox App >> . We recommend Bluestacks because you can easily find solutions online if you run into problems while using it. Download Bluestacks PC software Here >> .

  2. Install the emulator:
    On your computer, goto the Downloads folder » click to install Bluestacks.exe or Nox.exe » Accept the License Agreements » Follow the on-screen prompts to complete installation.

  3. Using Docket on PC [Windows 10/ 11]:
    • Open the Emulator app you installed » goto its search bar and search "Docket"
    • The search will reveal the Docket® - Immunization Records app icon. Open, then click "Install".
    • Once Docket is downloaded inside the emulator, locate/click the "All apps" icon to access a page containing all your installed applications including Docket.
    • Now enjoy Docket on PC.

  1. To get started, Download the OpenPC software »
    • Upon installation, open OpenPC app » goto "Playstore on PC" » click "Configure_Download" button. This will auto-download and install PlayStore on your computer.

  2. To use Docket,
    • Login to the PlayStore we just installed
    • Search for "Docket" » then Click "Install".
    • Docket® - Immunization Records will now be available on your Windows start-menu.

Docket® - Immunization Records functions

- The app is fairly easy to figure out.

- The search screen does not indicate when records have been found.

- The app does not update records in a timely manner.

- The app does not display immunization records, only a date of update.

    My wife and I received our 3rd vaccine shots over 3 weeks ago. Still our records have not been updated. The app says the last records update was in early August. Refreshing does nothing. We got our shots at CVS and they have verified the state has the vaccinations properly recorded. You’re better off laminating your vax card and scanning it in. Update: one of the other reviewers mentioned that although the app doesn’t reflect the current vax record, it will show in the downloaded pdf. She’s right - I downloaded my pdf and shared it to my email, and it is current. Even that wasn’t working properly. When you hit the download button it just shows the spinner-busy forever. After about a half hour I hit download again and it miraculously displayed my pdf record. Not so much for my wife’s record. That’s been spinning for hours and no pdf. This app is a total fail - the idea was that it would be an easy and verified way to show your vax status. Nope. Also the page that displays the vax status has no identifiers for who it is so a person could very well be showing some other member of their family’s record to enter a space. Terrible app and the states should definitely look elsewhere for an app that works.

    Love the App! Most importantly I wasn’t able to get into the App prior an email I received! At first I thought it was a scam. I emailed back I’m not sending my ID you should have my info with the Statefrom the vaccinations for Covid19. I stated anymore info or to verify my info that your a rep. from the State plz call. I must say that you guys are on top of your game! I tried a few times & it wouldn’t work so I was going to try it the next day. The correction was made by Claudia Arredondo. I just realized I emailed you then she called me back! Okay anyway she was amazing! It was so easy my wife had to put in all the site codes ! Thanks again William Leahy
  • App works well after syncing

    The app was fairly easy for me to figure out and provided a clear path to fix the situation when my records weren’t found. I am guessing many people will need to use that method to reach out to their state health department and get all records synced up to their phone number and email so they can be found on this app. For me this took a couple of weeks, then I received an email telling me to log in to this app again. Developers: when I went back in and performed my search, it just stayed on the same search screen each time rather than taking me to actually see that records had been found. I assume this is on your backlog to fix. It meant that I kept re-running the search with no indication it had completed or found results. Eventually I tried backing out of the search screen and found my records.
  • Good idea, such a bad implementation

    I've been a software designer, developer and manager for more than three decades and I have rarely seen a technology concept that was such a good idea so poorly designed, built and tested. Seems like the marketing people, obviously new graduates from the Imagineering school at Disney, felt that animated squirrels were more important than useful error screens and functional test scripts. Experience with the app from my extended pool of Utah Friends and Family ranged from poor to exasperating with the median results being the ability to create an account, have the app find immunization records, report the same and universally never display them. Mostly showing a pretty blue page with nothing on it except "Immunization Records updated on" xxxx Date. And that's all. No pdf, no display, nada. Through probably 60 trials over the course of fifteen days of various combinations and permutations of app deletion, account deletion and anything else we could think of, that's all we got. Bupkus. We suspect the money folks allotted 80 percent of the dollars to squirrel design, fifteen percent to coding and five percent to quality assurance. Well, ya got what you paid for. Too bad, cuz you could have built a helpful and profitable product instead of an infinite rewrite morass. Sigh.
