2. The SPICT-App™ is a guide to identifying people with one or more advanced conditions, deteriorating health and a risk of dying for assessment and care planning.
3. If we identify people in a timely way, we can review their health care and wider needs and those of their family carers.
4. We can find out about ‘what matters’ to them, introduce earlier supportive and palliative care, and begin planning future treatment and care with them.
5. The SPICT-App™ helps clinicians decide if a patient is at risk.
